By accessing our website, booking an appointment, or receiving services at CleanSlate, you agree to be bound by these Terms and Conditions. Please read them carefully before using our services.

1. Services

CleanSlate provides professional laser and skin treatment services, including but not limited to:

Laser hair removal
Skin rejuvenation treatments
Cosmetic injectables (Botox, dermal fillers)
Acne treatment
Pigmentation treatment
Other aesthetic and dermatological services as advertised

All treatments are performed by qualified medical professionals in accordance with Indonesian healthcare regulations.

2. Booking and Appointments

2.1 Appointment Booking

Appointments can be made through:

Our website booking system
Phone or WhatsApp
Direct walk-in (subject to availability)

2.2 Deposit Requirement

To secure your appointment, a deposit of IDR 500,000 (five hundred thousand Indonesian Rupiah) is required. This deposit:

Must be paid through our online deposit form at https://cleanslatelaserclinic.com/deposit-form
Will be deducted from the total cost of your treatment
Confirms your commitment to attend the scheduled appointment
Is processed securely through Xendit, our third-party payment processor

Important: By submitting payment through our deposit form, you authorize Xendit to process your payment on behalf of CleanSlate Laser Clinic.

2.3 Cancellation and Refund Policy

Cancellations made 48 hours or more before the appointment:

Full deposit refund or credit toward a future appointment
Refunds will be processed within 7-14 business days

Cancellations made less than 48 hours before the appointment:

50% of the deposit will be retained as a cancellation fee
The remaining 50% can be credited toward a future appointment within 3 months

No-shows or same-day cancellations:

The full deposit will be forfeited
No refunds or credits will be issued

2.4 Rescheduling

You may reschedule your appointment once free of charge if done at least 48 hours in advance. Additional rescheduling requests may incur a fee or require a new deposit.

3. Payment Terms

3.1 Payment Methods

We accept the following payment methods:

Cash (IDR)
Bank transfer
Credit/debit cards (via Xendit)
Digital wallets supported by Xendit

3.2 Pricing

All prices are listed in Indonesian Rupiah (IDR) and are subject to change without notice. The prices quoted at the time of booking will be honored for that specific appointment.

3.3 Payment Processing

Online payments, including deposits, are processed through Xendit, a licensed third-party payment service provider. CleanSlate does not store your payment card details. By making a payment through our website, you agree to Xendit's terms and conditions and privacy policy.

4. Medical Consultation and Treatment

4.1 Consultation Requirement

All first-time clients must undergo a consultation before treatment. During the consultation, our medical professionals will:

Assess your medical history
Evaluate your skin condition
Discuss treatment options and expectations
Explain potential risks and side effects

4.2 Medical History Disclosure

You must provide accurate and complete information about your medical history, including:

Current medications and supplements
Allergies
Previous cosmetic treatments
Medical conditions
Pregnancy or breastfeeding status

Failure to disclose relevant medical information may result in treatment refusal or complications for which CleanSlate will not be liable.

4.3 Informed Consent

Before any treatment, you will be required to sign an informed consent form acknowledging that you:

Understand the nature of the treatment
Are aware of potential risks and side effects
Have had the opportunity to ask questions
Consent to the treatment voluntarily

4.4 Treatment Results

Treatment results vary by individual. CleanSlate makes no guarantees regarding specific outcomes. Results depend on various factors including skin type, age, lifestyle, and adherence to post-treatment care instructions.

5. Age Requirements

Clients must be at least 18 years old to receive treatments without parental consent. Clients under 18 years old must be accompanied by a parent or legal guardian who will sign consent forms on their behalf.

6. Health and Safety

6.1 Contraindications

Certain medical conditions may prevent you from receiving treatment. We reserve the right to refuse treatment if:

You have contraindications for the requested procedure
You are under the influence of alcohol or drugs
You have active skin infections or conditions
The practitioner determines treatment is not in your best interest

6.2 Post-Treatment Care

You agree to follow all post-treatment care instructions provided by our medical staff. Failure to follow these instructions may affect treatment results and could lead to complications.

7. Privacy and Data Protection

We collect and process personal information in accordance with Indonesian data protection laws. Your information is used solely for:

9.2 Medical Complications

While we take every precaution to ensure safe treatment, all medical and cosmetic procedures carry inherent risks. CleanSlate is not liable for:

Adverse reactions to treatments when proper protocols were followed
Complications arising from failure to disclose medical information
Complications arising from failure to follow post-treatment care instructions
Unsatisfactory results when treatments were performed correctly

9.3 Third-Party Services

We are not responsible for any issues arising from third-party services, including but not limited to payment processing failures, website hosting issues, or booking system malfunctions.
